Examinations
Examination of oral structures including charting of teeth, soft tissues, periodontal examination. (For NHS patients this would mean an entry of a claim for examination.)
A maximum of 1 item for examination (if carried out) is allowed in this log per course of treatment.

From the very beginning to the end of your training year, as a Foundation Dentist you are expected to keep an accurate log of your clinical experience, and that includes the numbers of routine procedures carried out in practice.
The Clinical Experience Log lists and defines the procedures you will need to track on a daily basis when you are in your training practice treating your patients. Numbers of procedures completed is data that you will need to transfer from practice clinical records onto your E-Portfolio as soon as you are prompted to do so. You will not be allowed to delay updating this numbers log as it will be locked five days after the end of each month.

Please be truthful about the numbers you disclose. Painstaking accuracy in recording is demanded from you. This is because your TPD will be monitoring your workload as compared with your peers and looking for outliers. The TPD will have access to other forms of data from multiple sources that will aid in forming your profile. For instance data can be extracted from: NHS monthly pay schedules; dental management software; laboratory bills. Statistical analysis that highlights unusual data will signal your TPD to scrutinise information and pose questions surrounding probity.

Radiographs
A single radiograph that is of grade 1 or grade 2 standard counts as 1 item.

Impressions
An impression of an arch sent to a laboratory counts as 1 item.

Photography
Each usable clinical photograph that you take counts as 1 item.

Treatment planning- Adults (routine)
When you have formulated and agreed a treatment plan for a patient, that will count as 1 item.  For NHS patients, band 2 and band 3 courses of treatment require plans.

Treatment planning- Children in pain
For child patient who presented in pain (and not a routine recall appointment) requiring advice or treatment, you record 1 item for a course of treatment to manage the pain for the child patient. This course of treatment may fall under an Urgent course or Bands 1, 2 or 3

Treatment planning- Adult in pain
For an adult patient who presented in pain (and not a routine recall appointment) requiring advice or treatment, you record 1 item for a course of treatment to manage the pain for the adult patient. This course of treatment may fall under an Urgent course or Bands 1, 2 or 3

Patients with complex medical histories
You record one item for  each patient  who has a medical history which is of such complexity that if impacts upon the delivery of the treatment that you are providing for the patient.

Health promotion /disease prevention: diet analysis
For the purpose of this log, 1 item of health promotion per course of treatment is allowed if you have carried out health promotion in terms of lifestyle changes such as smoking cessation advice, alcohol reduction advice, dietary changes.
 

Treatment planning- Children (routine)
A child patient seen for a routine examination and receives a course of treatment under Band 1, 2 or 3 will fall in this category. One course of treatment is recorded as 1 item

Fissure sealants
The application of fissure sealants as a preventive measure, to one or more teeth in a course of treatment will count as 1 item..

Fluoride varnish
The application of fluoride varnish as a preventive measure, to one or more teeth in a course of treatment will count as 1 item..

BLS / medical emergencies training
A training session on BLS or medical emergencies counts as 1 item..

Medical emergencies
A genuine medical emergency requiring your intervention to treat the condition such as for a faint, asthmatic attack, epileptic fit etc.
Each episode counts as 1 item.

Dental trauma
When a patient consults you following an accidental or non-accidental injury to oral structures, this visit counts as 1 item.

Anxious patients
An anxious patient for the purposes of recording here will be defined as one who requires additional management techniques to control their anxiety. Each course of treatment for an anxious patient counts as 1 item.

IV sedation
Each episode of treatment under IV sedation counts as 1 item.

Local anaesthetic
Each appointment requiring a local anaesthetic counts as exactly 1 item only (irrespective of number of LA cartridges used).

Inhalational sedation
Each episode of treatment under inhalational sedation counts as 1 item.

6 point periodontal chart
A 6 point chart created for a patient diagnosed with chronic periodontal disease will count as 1 item. Multiple 6 point charts in the same course of treatment still counts as 1 item.

Simple scale
The simple scaling that is done, for example, in a NHS band 1 course of treatment. Count a maximum of 1 item for the provision of this treatment in any course of treatment.

RSD
The root surface debridement, biofilm disruption, or root surface instrumentation that you undertake as part of periodontal treatment that is done, for example, as part of a NHS band 2 course of treatment. Count a maximum of 1 item for the provision of this treatment in any course of treatment

Extraction of permanent teeth
The routine forceps extraction of permanent teeth. Each tooth you have completely extracted by only routine measures counts as 1 item.

Extraction of deciduous teeth
The routine forceps extraction of deciduous teeth. Each tooth you have completely extracted by only routine measures counts as 1 item.

Extraction of buried roots
Any root that is extracted needing the use of instruments other than extraction forceps, ie elevators, but not needing bone removal or raising of a flap. Roots extracted of one tooth count as 1 item - and only when extraction is complete!

Surgical extractions involving sutures
For the purpose of this log, any tooth that is extracted and required root division or bone removal, or raising of a flap AND suture placement, counts as 1 item on completion of this treatment

Prescribing (antimicrobials)
Prescribing a medicine for a patient that is an antimicrobial counts as 1 item for each prescription.

Prescribing (other)
Prescribing a medicine for a patient that is a prescription only medicine other than an antimicrobial counts as 1 item for each prescription.

Referrals
Referring a patient for advice or treatment outside the practice counts as 1 item .

Denture alterations and repairs
Adjustment, easing or repairing a denture (not as part of the course of treatment for the provision of that denture)  counts as 1 item .

Rubber dam placement
Each visit that you have placed a rubber dam on a patient counts as 1 item

Amalgam restorations
Each completed amalgam restoration (filling) in a tooth counts as 1 item

Anterior composite restorations
Each completed composite resin restoration (filling) in an anterior tooth counts as 1 item

Posterior composite restorations
Each completed composite resin restoration (filling) in a posterior tooth counts as 1 item

GIC
A permanent restoration of a tooth with a glass ionomer cement (GIC), or a restoration to stabilise a tooth (which is planned to be in situ for several months). Each completed restoration counts as 1 item.
Excluded are occasions when GIC is used as a temporary restoration and if you have had to replace a GIC restoration within the same course of treatment.

SS crowns on deciduous teeth
The placement of a preformed stainless steel crown on a deciduous tooth counts as 1 item.

RCT incisor / canine
The completed endodontic treatment of an incisor or canine tooth counts as 1 item

RCT premolar
The completed endodontic treatment of a premolar tooth counts as 1 item

RCT molar
The completed endodontic treatment of a molar tooth counts as 1 item

Veneers
A completed permanent indirect veneer (resin or porcelain) on a tooth counts as 1 item.

Crowns - conventional
A completed permanent indirect crown on a tooth counts as 1 item.

Posts
The completed restoration of a tooth with the use of a post, direct or indirect, metal or fibre reinforced, counts as 1 item.

Amalgam cores
The completed restoration of a tooth with the use of a core built up in-situ from amalgam counts as 1 item.

Bridge - resin retained
The completed placement of a bridge involving minimal preparation of tooth tissue and retained by the use of a resin bonding system counts as 1 item.

Bridge - conventional
The completed placement of a bridge using crowns or onlays as retainers counts as 1 item.

Acrylic complete dentures
Fitting a full upper denture made of acrylic counts as 1 item and also a full lower denture made of acrylic counts as one item.

Acrylic partial dentures
Fitting a partial upper denture made of acrylic counts as 1 item and also a partial lower denture made of acrylic counts as one item.

Cobalt-chrome dentures
Fitting a partial upper denture with a cobalt-chromium metal frame counts as 1 item and also a partial lower denture with a cobalt-chromium metal frame counts as one item.

Denture reline
Relining or re-basing an upper or lower denture counts as 1 item per denture.

Additions to dentures
The addition of a tooth, teeth or a clasp to an existing denture counts as 1 item per denture.

DNA numbers
This is the number of missed appointments that are recorded in your appointment diary.

Number of unbooked clinical hours
This is the total of the time allocated to see patients, but not used (e.g. cancelled appts, time not booked). This does not include time for tutorials or administration sessions.

Holidays
This is the number of days taken as leave during the current month

Detailed oral hygiene instruction
For each patient you advise about oral hygiene, including presence of plaque and plaque control demonstrations, you count 1 item.

Sick days
This is the number of days taken as sick leave during the current month
